Required items at the time of registration:

• Complete Final Forms
• Copy of certified birth certificate (not a hospital certificate)
• 2- Proof of Residency document. (acceptable items )
• Moving into the District or building a new home New Residency Requirement
• Parent/Guardian ID
• Most recent proof of custody if the child(ren) are not living with both natural parents.
• Child's Social Security Card/ Number.
• Current Report Card or transcript information.
• IEP/ 504 If Applicable
• Child's immunization records (Click here to view list of required vaccinations)
